CONFERENCE OVERVIEW

"Chronic Disease Care: Better Ideas for Solving Real World Problems" is a networking conference. It is designed to facilitate real information sharing--of practical ideas that are currently in action and can be used in a range of clinical settings. Sessions are designed for participation, not just passive listening. Participants will be expected to actively listen, share, question and learn throughout the two-day session. We encourage you to be a part of this experience and hope that you will bring your knowledge, know-how, beliefs and uncertainties to this forum.


LEARNING OBJECTIVES

At the conclusion of the conference, participants should be able to:
  1. Identify specific ideas to improve chronic care that can be adopted in their own setting
  2. Articulate specific strategies to support team-based and collaborative care
  3. Understand the importance of and describe practical ideas for supporting self-management
  4. Suggest appropriate steps to support chronic care from distinct perspectives (e.g., practitioner, payer)
  5. Be poised to adopt better idea(s) through clear understanding of what changes make a difference
  6. Assess the applicability of adopting information technology to support chronic disease care in their own setting
  7. Take next steps towards making change (and transformation).

CALIFORNIA IMPROVEMENT NETWORK

The California Improvement Network is a project of the California HealthCare Foundation that seeks to strengthen, coordinate and expand improvement programs across the state. One of the key ways of supporting clinical care improvement is to provide opportunities for practical information sharing. This conference is designed to articulate the practical steps required to improve chronic care and network those who are making (or supporting) these changes.
