This session is open to any participant, but will require complete and focused engagement (no cell phones, pagers, etc.). Doors will close as the session begins.
So what would care be like in our communities if we could redraw the maps, disregarding some of the known boundaries? What new spaces and places might we be able to design so those with chronic and long-term conditions might better be able to care for themselves, or be cared for by others? How might we achieve this, building on and extending on the ideas we have learnt in the conference?
